Church Event Proposal Form

Please use the button below to submit an event proposal for any ministry event at MFC that is open to the public, the full congregation, or a specific group (Children, Youth, Men, Women, etc.).

All proposals must be submitted at least 60 days prior to the event date.

Please note: small group events (for a single small group) are not considered church-wide events and do not require this form.
